Founded in 2005 to provide investigation services into the failures of operations within critical buildings and establish the cause of the failure and to make recommendations to prevent the recurrence of the failure and to feed the failure back into the design of the critical building/facilities design. We need to remember that some 90% of all failures have direct human involvement.

People are critical in building the most valuable asset and the largest liability.
